Well leaders of smaller villages are usually on the level of a kage despite the fact that the village is smaller. So i just thought the title was a generalization.

Watch the episode where Kakashi explains that there are only 5 people with the title "Kage" and they are for the 5 main elemental countries. But it doesnt matter because the leaders of the other villages are usually just as good as kages.
